HEALTH INSURANCE Milwicz Insurance Group, Inc. offers individual and Family Health Insurance Plans;
Co-pay Plans, High Deductible Insurance Plans and Basic Plans, Health Savings Accounts (HSA’s)
Individual and Family Health Insurance Plans-
• Co-Pay Plans- Designed for individuals and families who want help managing their routine medical expenses,
plus coverage for major health care expenses. Three levels of co-pay- $25, $35, $45 with deductible and co-insurance options.
A Preferred Provider Organization (PPO) Plan; choose your primary care physician from an extensive list of general practitioners,
internal medicine physicians and specialists.
After deductibles and coinsurance the plan pays for; Doctor office visits, Doctor office X-ray and Lab, Preventive Care,
Mammography, Pap Smear, PAP Testing, Outpatient Prescriptions, Inpatient Hospital and Surgical Fees included in the Hospital Bill,
Outpatient Surgery and Surgical Fees and other covered Inpatient and Outpatient Fees, and Emergency Fees. A maximum Lifetime
Benefit Option of $3,000,000 per covered person, an initial Rate Guarantee of 12 months- subject to benefit and address changes.
Optional benefits include Maternity, Supplemental Accident and Term Life Insurance.
• High Deductible Health Insurance Plans- For individuals and families willing to take additional responsibility for
routine health care expenses in exchange for lower premiums. Choose between a Preferred Provider Organization (PPO) and non-PPO.
Two plans- 100% after deductible and 80% after deductible with two coinsurance options.
Plan 100 pays 100% of the charges after the deductible for; Doctor Office Visit Fees, Outpatient X-ray and Lab, Preventive Care,
Mammography, Pap Smear, PSA Testing, Outpatient Prescription Benefit, Inpatient Hospital and Surgical Fees, Outpatient Surgery
and Other Covered Inpatient and Outpatient Fees, and Emergency Fees. A maximum Lifetime Benefit Option of $3,000,000 per covered
person, an initial Rate Guarantee of 12 months- subject to benefit and address changes. Optional benefits include Maternity,
Supplemental Accident, Term Life Insurance Benefit and a Prescription Drug Card.
Plan 80 pays 80% of the charges after deductible. Benefits same as Plan 100 except the coinsurance option pays 80/20 to $5,000-
then pays 100%
Basic Plan For individuals and families willing to take responsibility for routine health care expenses in exchange
for basic premium rates. Choose between a PPO and non-PPO. Five deductible options from $500 to $5,000, maximum 2 per family
per year. Coinsurance pays 80/20 to $10,000, and then pays 100%. Plan pays 80% after deductible for; MRI and CAT scans, 80%
after deductible for Other Tests Performed Within 14 Days of Surgery or Confinement, 80% after deductible for Mammography,
Pap Smear and PSA Testing, 80% after Deductible for Inpatient Hospital and Surgical Fees, Outpatient Surgery and Other
Covered Inpatient and Outpatient Fees. Lifetime maximum of $3,000,000. Optional benefits include Maternity, Supplemental
Accident and Term Life Benefit.
• Health Savings Accounts (HSA Plans)- Help you take control of your health care expenses with a tax-favored savings
account and quality medical coverage for individuals and families. Take the money currently spent on existing health insurance
and put a portion towards a qualified higher deductible plan, with the remainder going into a tax-favored savings account.
HSA’s are tax deductible- contributions to the HSA are 100% tax deductible: HSA’s are tax favored- withdrawals for covered
medical expenses are never taxed: HSA’s are tax deferred- interest earnings accumulate tax deferred, and if used for covered
medical expenses are tax free. Savings not spent in any year remain in the HSA; tax deferred and can be used in subsequent years
for covered medical expenses.
